Updates to the Forms - Maintenance, forms, DVIR forms, rules, and notifications

New features to be introduced to Forms:

1. Creation of repair requests from the form submission

2. Creation of repair requests from the DVIR submission

3. Show/hide fields in the form based on the data entered by submitter

4. Notification of form submissions

5. Send form as a task based on rule triggers

6. Sending form for approval to the supervisors

7. More automated data capture such as Odometer, location, etc.
